Ask your building for a sample certificate or a written list of their requirements, and send it to all three of the addresses opposite — both fax numbers and the email. Buildings vary in what they want named and at what coverage, so the requirements themselves are what we work from.
Include your name and apartment number. Without them the certificate cannot be tied to your job, which is the most common reason one comes back rejected.
If your building's managing agent would rather send the requirements directly, that is fine — they can use the same fax numbers and email.
